Intensive services focus on maintaining permanency to avoid of out-of-home placement when appropriate. If out-of- home care becomes necessary this service will support youth and families in reunification once placement is terminated.
The Preventative Aftercare Intensive program is flexible and individualized to accommodate the specific needs of the youth and family. Assessments are completed to identify strengths, needs, abilities and preferences. This becomes the basis of the individual treatment plan which is developed collaboratively with the youth, family and involved agencies. The goals of the treatment plan are specific, measurable, attainable, relevant, and time-specific to the youth and family. Services are provided in an intensive manner three to five days a week through individual and family therapy and case management activities with the ultimate outcome of keeping the family intact.
